BSA Medical Forms

We want these on file anyway, and they are required for Summer Camp and the Summer Trip.

Seriously. They literally will not let you stay at camp without one.

Note: You fill out Part A and Part B yourself, and it’s pretty easy.

You have to get Part C done by your doctor. Pediatricians are used to this; they do it for camps, teams, and other activities all the time. Just drop it off at their office in an envelope with your Scout’s name and your phone number on it.

They typically ask for 10 days or two weeks to turn it around, so do not wait until the last minute. (In practice, they usually call you a day or three later to pick it up, but still.)

Some just return it stapled to a print-out of the kids’ vaccination schedule. That’s fine, so long as there’s a signature scrawled in the right spot.

We recommend immediately making a copy before handing it over to Tom Finley for scanning and filing.

Background checks & Adult training modules

Any adult who wishes to accompany the troop on an overnight—even their own kid—must join Scouting America first. This is a safety/youth protection thing, and since safety is our #1 goal, we agree. Despite it being a bureaucratic headache and minor time-suck.

That mean’s you have to sign up, and then submit a series of background checks.

All adult volunteers are also required to take the Safeguarding Youth online training program (what we used to call YPT). It will be available in your account on my.scouting.org once you are signed up.

If you want to have a more active role as a Troop Committee member or Assistant Scoutmaster, there are some training modules to do (particularly for the latter). It’s all in your account on my.scouting.org.

Letter of consent

This is for summer trip travel—especially international trips travel. It needs to be signed by all parents/legal guardians—even if one parent is on the trip.
